Bad Throwout Bearing?

Is this a throwout bearing issue, clutch issue, slave cylinder issue or something else?

My clutch is no longer full engaging and I can't shift. In the video you can see the bearing is favoring the top of the clutch and not full engaging on the bottom. If I push the clutch fork upward by hand it engages the clutch more evenly and I can shift.

The engine is a '96 Jeep 4.0 mated to a NP435 Transmission. I think the clutch is from the jeep. Throwout bearing is a year old.

 
That does not look correct to me. The throwout should be riding on the input shaft retainer and not allowing it to twist/angle sideways. Maybe fork cracked and allowed it to wear the retainer on one side or retainer is damaged. Hard to say without pulling it apart. I would just drop the trans and check it out thoroughly.
