If he just removed the nut, wouldn't it just spin on the finger? or did the digit destroyer switch the drive and screw the bolt onto his finger?
He hit the impact and screwed the nut onto his finger.

Suffice to say if the thing went left it wouldn't have done that. So logic would dictate it went right unless it was a left hand thread. In which case any point concerning direction is moot. It's on his finger in a bad way. That's all we can see here. The guy got screwed. Pun intended.

Best course of action would be to have immediately reversed before it started to swell. Bet he wasn't thinking that quickly.

Unless the knuckle got crushed or bones broke. In which case screwing it back off might actually tear/rip his finger off. Otherwise, that is the best course of action.

I generally run it wot and tap the side of the socket on something which results in the nut flinging off into oblivion
 
Wet saw it off (ice water bath) with a die grinder and wheel of death. That'd be my removal choice. When close to skin underneath nut, after cutting opposite sides, finish with a nut splitter. You Use an impact on the nut splitter if you're a real man.

Or just heat it up with a torch til it expands enough to fall off. Bonus points for the skin not even hurting when you're done.
