04 grand Cherokee 4.0 weird noise

A coworkers jeep has a weird noise. Almost sounds like something loose inside engine when he first starts it, but it quietens down and idles normal. Every time we rev it there is a loud click or pop that sounds like it is coming from the throttle body area. It continues to make the sound until you let off the throttle. Any ideas?
 
My first thought based on the location and going away once warmed up would be exhaust leak. Cracked header/exhaust manifold?
 
I think it is a backfire into the intake. at least that is what it sounds like to me.

a little more background. It has been doing this for a while, but has recently gotten worse. not too long ago, the battery went out and had to be replaced (battery was less than 3 years old at the time). Engine was doing this before the battery went out, and ran fine with the new battery installed. Over time has gotten worse again. I beleive it could be a fuel or timing related issue due to low voltage. I'm at work and have no way to test anything, he already has a tow truck on the way.
