i replaced the right side caliper and pads after near death from a bulldozer, so mine pulls real hard to the right. Maybe the first owner did something similar. Check the pads on the left side maybe they are more worn
It's the brace on the front axle. Can't remember what it's called but look under the front end. a bar that goes from the front axle to the frame. The bushings are worn out in it. When you step on the brakes the front axle wraps and because of the push/pull steering it steers (pulls). You can replaced the bushings or you can lift the truck with better springs. Also NEVER replace just one side of the brakes. Go back and replace the pads on the other side. You can do just one caliper, but always do brakes on both sides.

90 4runner, when does it smoke? Brakes applied, or after driving it without using the brakes much? It could be a locked up caliper, if it smokes all the time, but odds are it would pull a bit all the time, not just during braking. Also check how loose is your steering? If it does turn out to be a caliper, now is the time to upgrade to vented rotors. Get rotors off a '80s land cruiser and the calipers and pads off an '86 or so 4runner. If you get calipers off a V6 version, you'll need the master cyl too. Easy way to tell, the calipers off a 4 cyl model will have 2 different sized pistons, and a V6 one will have two the same size.
