Accidental injury report?

Anybody ever had to fill one out work just sent me one. Not sure if insurance is trying to get out of paying a claim what should I do? Any advice?
 
Fill it out and be honest, list all witnesses. It's procedure for documentation.
 
Not sure of the details, but for things like that, I usually ignore the first request, wait til I get asked again.

And personally because I'm the paranoid type, while I waited on the 2nd request Id probably use a free consult with a lawyer.
Circumstances depending of course. Are we talking hundreds of dollars injury or thousands of dollars of medical expenses injury.
 
I dont know about your guys experience. But where I work, workers comp claims have been denied due to not being reported properly. Then the guy got fired.
 
No that's the thing it's not workers comp. it's from me hitting my head at Busco. The paper said to return with 30 days or I would be charged the full amount which is around 2100 dollars. I'm hopeful that this is just clarify there isn't a 3rd party liable for my accident. But I don't trust anyone especially my work.

Well, I think you have answered your own question. Unless you want to be charged said amount then figure out a way to not have to pay it.
 
Yea I've done lots a shit never had to fill this out though. I've filled it out hopefully this will be the end of it
 
I did one a couple weeks ago for my f tup thumb. I just figured since mine was a bicycle wreck, they were trying to see if there was any way to get car insurance to pay for it.
 
What am I missing? Why is this so bad, or there be any reason to not be completely open and detailed about it?
It soudns liek you got hurt at work, there's already been some expenses incurred, HR wants full details on what happened. The only reason I can think of NOT to provide full details is if you were doing something you should not have and caused the injury.

Checking in w/ a lawyer may not be a bad idea but I suspect you're going to have to fill it out either way.

Honestly I want doing anything wrong. I've just never had to file any paperwork like this .
 
I just went through this with my knee injury from a bike wreck they want to make sure it was totally your fault and no one else was involved. Just state what happened and that you were the only one involved and you'll be fine.

I don't get involved in shinanigans, even mature responsible adults have accidents some times :eek:.
