my 3.5 year old son has this divot in his neck that we can’t tell what it is? It’s been there since birth, but it has been oozing a small amount of puss more and more lately.

Is this something to be concerned about, and what is it called?

Edit: his last time at the dr, she looked at it and asked “has this been there since birth?” Wife said yes and she(Dr.) just replied “ok” and that was it

If it's oozing puss, go to the Doctor.

I vote with the different doctor, might be something simple or very serious. (I am NOT a doctor and CANNOT give medical advice)

Could be pyoderma may want to get that checked sooner than later especially if from birth it’s had some time...

What is a pyoderma? Derma I understand as being Greek for skin, the prefix (pyo) I have no idea what it means, too late in the evening to look it up in the dictionary.

Reason this has come back up, was the baby sitter told us it was getting worse today. Apparently he has been picking It off during the day and it would not ooze/scab back up for days or weeks. So we only saw it scab once in a while.

It hasn’t been oozing since birth it probably started 12-18 months ago.

year? doctor. Maybe just a cyst. But letting it go means it possibly getting larger. This means more scaring to remove.

Either or puss means infection, infections that linger means doctor.

And some backstory:

Noah was born 7weeks early due to my wife having very severe preeclampsia.

His lungs were not fully developed yet and he spent his first 25 days in the NICU due to having bradys . I wonder now if it is something that didn’t form completely?

I spent over 70 days in house and had all my insides hanging out.....had pin sticks and tubes coming out all my appendages and my head. I got a scar on my neck I can feed a needle and thread through in two places cause the stitches stayed so long.....they where more concerned with the zipper across my entire belly.

But at 3.5 year I wasn't oozing puss from any of it...get that youngin looked at. could be a mild form of anything waiting to go nasty.

We are making an appointment tomorrow.

Obviously not with his reg doctor.

Dermatologist?

Pyoderma gangrenosum (pie-o-DUR-muh gang-ruh-NO-sum) is a rare condition that causes large, painful sores (ulcers) to develop on your skin, most often on your legs.

No one knows exactly what causes pyoderma gangrenosum, but it appears to be a disorder of the immune system. People who have certain underlying conditions, such as inflammatory bowel disease or rheumatoid arthritis, are at higher risk of pyoderma gangrenosum. Pyo if I recall from my 4 awesome years of Latin means pus “pūs“ origininates from Greek pyon!

I'm not a real doctor (as my kids often say).
Entirely possibly it's a sebaceous cyst, which is just a little fat pocket under the skin. Sometimes they happen and are completely dormant, then begin to fill with puss and will either balloon up or ooze.
I had one right in the middle of my chest, when I was a teenager it showed up randomly, looked like a tiny BB under the skin.
Fast forward 10 years, it randomly started growing, over the course of a few weeks grew to the size of a grape. Dermatologist lanced it just to get the nastiness out, but had to remove the underlying pocket (like grape skin) to prevent it coming back. Left a huge scar b/c it had grown so much.

Also had one on my wrist once, that one was just scalpeled off. No problems since.

A dermatologist will deal with this properly. Unlikely to be anything serious, but if not dealt with could mean an issue/scar later.

I've had 2 sebaceous cysts removed over the years and a pilonidal cyst that I waited way too long to get it cut out. I have a small sebaceous cyst in about the same place as the blemish on your sons neck, closer to my adams apple. I used to squeeze it now and then but it's just a white, pea sized lump now and has not gotten larger or had any discharge in a few years.

I am not a doctor, but I play one on television...

In all seriousness, if it were my kid, we would be seeing a different doctor at the very least, and probably going to see a dermatologist.

There's no reason to see a "different" doctor. A specialist, sure. But let's assume for a second that it's a cyst - the last time the Dr saw it, it was fine, no big deal. Now it's infected. That's not a knock on the doctor, and no reason to go finding a new one.

Continuity of care is important, and shouldn't be overlooked.

Agree.
From the pediatrician's standpoint, things that are innocuous and been there since birth, and look like a normal cyst, are really no problem.
The only thing they really could have done any better is to have said, "If it changes, let me know and we'll deal with it." But everything from then to now would have been the same anyway.

So go see a dermatologist, then go back to your normal guy at the next scheduled checkup and let them know what happened so they can add it to the records.

Did he have a tracheostomy by chance when he was a baby or perhaps some other sort of tube inserted there?

The reason I ask is that my brother had a poorly performing kidney when he was born and he had several surgeries trying to correct it. One of the incisions healed up funny and he had a spot on a scar that would fester a little, never as much as what your son has though. The Dr's always kept an eye in it but never did anything about it. It finally went away on its own in his teenage years.

This is one of those risk reward conversations.
Will you drive your nice unmolested FJ40 across this rickety ass bridge over a 1,000' ravene? If you make it you have to turn around and come back. If the bridge fails...

You go to the doc. At worst you are out $50 and an hour and you get peace of mind. At worst here is something wrong and you start treating it as soon as possible.

I'm not a doctor, but I'm generally smarter than my friends that are. If your current doc has no idea that it's changed, you could start there. But that probably costs the same as a dermatologist visit, and the dermadoc should be much better trained in diagnosing and treating it. If it needs to be cut out, they can probably do it right then and there instead of having to schedule a second appointment.

BTW, What year was the picture taken and what type of potato did you use to take it?

Last night..?

And It’s an I-potato 5 or 6 can’t remember. Not that it’s a great camera but getting him to stay still was not easy
