2) Can replace the rear outer bearing without removing the t-case and taking the entire thing apart? The rear outer bearing is #4 in this picture. (bottom center of pic)

Not sure if you can replace it that easy. Andy at CRS rebuilt mine after I had failure on lower 2. I think bearings is what caused my troubles. And yes, that is too much play. I say fix it soon to keep your gears. It's also a good time to do the 4.0:1 swap.
 
yes, way to much end play... you can definitely replace the outer bearing without removing the case.. I wasn't happy with my shims/endplay when i did my gears on the bench, so I needed to fix that in the jeep... (Part#20)

The rear bearing cone will come out as soon as the housing pulls back enough from the shaft. The race is set in the tail housing.. Just install the new race and the new cone when your putting it back together...

I didn't pay attention last time but the hole is be enough you might be able to pull the inner bearing (cone) off the shaft inside the tcase - the race will be in the rear housing so at least inspect that for wear..
