on a full hydro steering setup dose there have to be two pistons coming out the ram or will one work.

You can use double ended or single ended ram, but either have to be double acting.

it is a single ra used for ram asst but it has 2 hoses that comes out my steering box

Only real difference between assist rams and full hydro rams, is the size. It will likely be to small for full hydro, but it's hard to say without knowing how big it is
