Hitch plate

Anyone know of a place that can make a hitch plate? I found a place online but it's in Ohio.

Can you clarify a little more what this is? Is it a plate to mount on the hitch, or a plate welded to the bumper to mount a hitch receiver on? Curious.
Looks like something about any fab shop could do.
