Is my receiver loose?

Is this how the mounting bolts for my receiver and bumper are supposed to be, or is this plate supposed to be tight up[ against the frame? Ive got the nuts very tight, but I cant get the plate on either side to go up against the frame. Should I smack them with a 5 lb sledge?

If you're talking about the rusty plate that holds the bolts, it's fine, the key is that the reciever and the plate that slides inside the frame are tight against the rail.
 
Is that metal sleeves on the bolts? If not, where are the threads on the bolts? Are they bottomed out which makes the nuts tight against the bottom of the thread rather than the frame??
 
The bolts slide through the sleeves. Im wondering why the hole on the inside of the frame (pictured) is so much bigger than the sleeves? Shouldnt it be a snug fit?

My guess is the metal sleeve the bolt goes through (between the frame rail) is a little long, keeping the plate from making contact from the inside frame rail. That said if the sleeve is of larger diameter than the frame hole on the out side, then essentially it's still working the way it's set up. Not optimal but if the sleeve is pinching that outer frame rail then it is working.
