threw rear driveshaft.. AGAIN

Threw out my rear driveshaft for the second time this year in my 250.
First time: looked like one of the bolts on one of the u-joint straps on the yoke vibrated out, causing the shaft to let fly.
Today: broke a head off one of the bolts on one of the u-joint straps on the yoke, causing the shaft to let fly.
Both times happened while starting from a red light, not goosing it or anything, just casual driving.
Wondering if both instances are coincidental or if there is a cause to the problem?
Also the best method of getting the broken bolt out, just drill and tap? helicoil? ....?
If it makes a difference the rear end is your run of the mill 10.25 w/ limited slip & 4.10s
 
Do you have the u-joint strap style or u-bolt style attachment on the yoke? I'd probably think about converting to a u-bolt style yoke or drill yours out to put u-bolts in if you're having problems with the straps (if that's what you have).
 
Check for axle wrap?
 
or change the straps when you pop in a new joint. Also if the previous breakage has messed up the yoke you may need a new one. In which case upgrade if you can. Otherwise, the straps can stretch over time and are pretty cheep.
 
No lift on the truck
Axel wrap has been on my mind lately, I have a tuner chip and a modified injection driver module. I know its not pushing any big HP to speak about but I feel it might be enough for axel wrap.
I will look at getting new straps, changing to u-bolt style sounds like the best bet right now.
 
are the rear springs worn? sag in the rear could cause poor driveline angles comebine that with axle wrap and youve got something to worry about.
 
I had a xj that would brake them like crazy. I went to the u bolt style and never had anymore problems.
 
Ugh, the bolt broke off half way down into the yoke and it started raining. Going to take the yoke out tomorrow or wed. to put it in the drill press to make sure I drill straight through the bolt.
 
well I couldn't ever find anywhere that sold u-bolts to fit the yoke, or anywhere that could make any, no one could make a bend that small (1-3/4"). Back to the straps, bought some grade 9s for them though
