The articles say she was sitting in the van when the rope broke then recoiled severing the arm. There were two ropes according to the report, one attached to the front of the van, the other rope that severed her arm was apparently wrapped through the windows and around the a pillars for some reason.

Sounds like something dumb was happening and they "made" a tow point by running rope through the windows.

Wow!

I saw something on Mythbusters that showed unless a rope/cable was wrapped around something it would almost never break and whip. I guess going thru the window allowed for a radius to whip around.

Look at how thin the rope they are using is. Check the front edge of the door.
