Toyota Steering box

How do I tell if my steering box is going bad? Steering is getting loose and kind of sketchy, I have to hold the steering wheel right to keep straight sometimes and other times have to hold it left to keep it straight, the only time the steering wheel is straight like it should be is when I'm about at a dead stop....figured this might be the issue any suggestions are greatly appreciated

Just bc there so wide? Or what? I plan on going back down to a 15" wide tire
