It means be upstream so if you lose the paddle you can drift back down. It is about being on the safe side or being ahead if something bad happens.

I think up or down is irrelevant. It's just a figure of speech. The point is that you're in a creek of shit with no means to navigate to safety.
 
I dont think I'd want to be in any creek full of shit.... paddle or no paddle.... boat or no boat...
